Weekly coaching check-in: a 10-question template

The 10 questions of a weekly client check-in for fitness coaches and nutritionists, the answer type to use and what each question tells you. A template to copy.

A weekly check-in is a short questionnaire a client fills in every week so their coach can see their measurements, energy, sleep and plan adherence, and adjust the programme. It replaces the "so, how was the week?" sent on WhatsApp, whose answers cannot be compared from one week to the next.

A good check-in takes under five minutes. Past ten questions, clients answer less often and less carefully. Here is a ten-question template, with the answer type to use and what each one tells you.

The 10-question template

Why these answer types

Numbers for what can be measured. Weight and waist must be numbers, not free text, or you cannot draw a chart. One week says nothing; six weeks show a trend.

Sliders for what is felt. "Fine" cannot be compared. A 7 out of 10 last week and a 4 this week can. Keep the same scale every week.

Choices for adherence. A closed question with four answers is more honest than a made-up percentage, and it takes a second to fill in.

Text only at the end. Two open questions are enough. They are often the ones that reveal the real problem: a trip, a hard Ramadan, an injury.

The rules that get clients to answer

  1. The same day every week. Sunday evening or Monday morning, to review the week just gone.
  2. Weigh in under the same conditions. In the morning, before eating, after the bathroom. Otherwise the differences mean nothing.
  3. A reply from the coach within 48 hours. A check-in with no reply is the last one the client fills in carefully. One sentence is enough: what you saw, what changes.
  4. One reminder, not five. An automatic reminder after the due date, then a conversation if the client still does not answer.
  5. Compare, do not just read. What matters is the change from last week, not the answer alone.

Adapting the template

  • Nutritionist: replace question 6 with water drunk per day (number of glasses) and add cravings (yes or no).
  • Performance coaching: add perceived session effort (RPE, 1 to 10) and soreness.
  • Weight loss: add hip measurement, taken at the same spot every time.

For progress photos, always ask for the client's consent and tell them where the photos are kept. Suivi does not collect body photos today.

Doing it in Suivi

Suivi sets up this check-in without copying anything out:

  • start from the built-in Weekly Check-in template (body measurements and wellness) and edit the questions;
  • add a measurement (weight, waist, energy, sleep, stress…) instead of typing the question: it arrives with its unit and its answer lands on the client's chart;
  • schedule the check-in weekly: the client gets it on their phone and answers one question per screen, in French, English or Arabic;
  • a reminder goes out automatically after the due date, and a series left unanswered three times in a row pauses and tells you;
  • your check-in inbox sorts what needs review, is overdue or is awaiting the client, shows the change from the previous check-in for every number, and your reply appears under the client's answers.
